Patients scheduled for their first visit—or who have not been seen in 12 months or more—are asked to complete the Patient Information Forms prior to their appointments. They can be printed or submitted electronically. These forms help ensure that essential information is entered into your record for proper treatment and billing. INSURANCE & BILLING “FYI”
Denver Osteopathic and Sports Medicine Center accepts most major medical plans except Kaiser, Medicaid, OptumCare and TRICARE. (NOTE: Dr. Cooperman IS STILL IN-NETWORK with UNITED HEALTHCARE.) Please verify that osteopathic manipulative therapy is a covered benefit under your policy and if there is a limit on the # of visits you are allowed! It is the patient’s responsibility to obtain a referral, if required by insurance. We also offer discounted fees for uninsured, self-paying patients.
